w-9 forms

I signed with onlyfans recently, but before setting up banking info they have you fill out a w-9 form. i dont really recall doing this with the other 5 sites i signed with over the years but i must have at some point?? well i want to be sure im filling it out the right way the first time to avoid having to jump hoops to correct it later on. im not asking for tax advice, simply wanting to verify im doing this legit.

line 1: is my name, cant mess that one up lol
line 2: business name: do i use my true name or my online persona?
line 3: this is where im actually confused. i didnt apply for any licensing of my brand. do i check "other"?
line 4: what?? lol i am inside the u.s, do i skip this or enter 0 or n/a?

Line 1- Your legal name, yep!

For Line 2- Don't fill out a business name unless you have a registered LLC or S-Corp or something like that that you're doing business under! If you're just getting check/deposits directly to you, leave line 2 blank.

Line 3- You're (I assume, if the above is correct and you haven't registered a biz) working under your own SSN so you would check the first one, Individual/Sole Proprietor etc.

Line 4- Skip this!

But DO fill out your address in 5 and 6, that's how you'll get your 1099s next year for your taxes.

Also, don't forget your SSN in Part 1.
