My girlfriend and I created separate accounts but we always go on together usually on her account. How can we change her account to a joint account for us both? Thanks a bunch!!!

MFC doesnt have joint accounts so just keep both open to be both verified on MFC and you're fine.
SM has separate couples accounts and you need to apply for a new one to get that. You cant merge accounts or anything, it has to be brand new. Depending on if you wanna keep your followers and such it may just be better to keep camming on one of yours.

I was hoping to pay to get a template built for us that would include both of our photos. I thought at one time I saw some girl have an account for her and another girl. Thank you!
For MFC you can absolutely use an account to cam with one or multiple girls. MFC requires all girls streaming to be verified though. They dont offer separate couples/joint accounts. So if you wanna use hers as one you absolutely can but you must keep your account open and in good standing (Verified ID basically.) But you can post pictures of your both on hers and even sell vids and such, there's no problem there.
