W-9 SSN Validation Failure

So I got an email on august 2nd about how they didn’t have a W-9 on file for me, so I filled it out and sent it in. Then this morning I got another email about SSN validation failure so I printed out the form and filled it out and got the same exact email. So am I filling it out wrong? What box do I check on line 3?
